Corneal Collagen Crosslinking
The procedure of choice to stabilise keratoconus and other corneal ectasias (conditions where the corneal shape becomes unstable).
Summary
The cornea is the clear front window of the eye, and a normal shaped cornea is vital for clear eyesight. In Keratoconus and similar corneal conditions, the cornea becomes thinned and distorted, affecting clarity of vision, and getting progressively worse over time. Collagen cross linkage stabilises the cornea and prevents further deterioration, using a combination of the vitamin Riboflavin and ultraviolet light. In some cases it can produce a modest improvement in vision, although stabilisation is the aim. By performing collagen cross linkage, we hope to be able to prevent future deterioration of eyesight, and avoid the need for major surgery such as corneal transplantation. Collagen cross linkage can also be used to “lock in” visual improvement from certain other corneal procedures such as excimer laser and corneal implant insertion.
The Process
One eye is treated at a time. The procedure is done in the operating theatre, under local anaesthetic (drops into the eye).
Some of the skin is gently removed from the surface of the cornea, and the cornea is then soaked in Riboflavin (vitamin B2) solution, a natural vitamin which is essential for corneal strength, for 10 minutes.
A controlled dose of ultraviolet light is then shone onto the cornea for 10 minutes.
At the end of the treatment, drops and a soft contact lens (bandage lens) are applied, and pain-relieving tablets are given.

Risks
Collagen cross linkage is highly successful but for 2-5 days following the procedure, many patients experience moderate to severe pain. We can support them by careful advice on pain relief, and daily healing checks until pain settles. Other risks include:-
Infection
Incidence about 1 in 100. Could require intensive antibiotic treatment and rarely result in a corneal scar and permanent loss of clarity of vision.
Slowness to heal
Occasionally the corneal skin can be slow to heal causing prolonged blurring of vision and irritation for up to a few weeks.
Change in focus
The spectacle or contact lens prescription required for clearest vision may alter, so that a change of glasses or contact lenses is needed. The prescription strength may increase or decrease.
Toxicity of ultraviolet light
Overdosage of ultraviolet light to the eye could cause permanent clouding of the cornea or a cataract, both of which might need surgery. This would be extremely rare, and due to equipment malfunction.
Corneal hazing
The cornea may sometimes stay slightly hazy after cross linkage even though it has all healed up. In rare instances this can permanently reduce the clarity of vision.
Progression of ectasia
In a few cases, the cornea is not stabilised fully and we detect further deterioration over time. In such cases, repeat treatment may be recommended.
